Indications

• Specific indications where topical corticosteroid therapy is considered the best treatment : contact eczema, atopic dermatitis, lichen.
• Indications where corticosteroid therapy is one of the common treatments : seborrhoeic dermatitis (except the face), psoriasis, lichen, stasis dermatitis, non-parasitic prurigo, lupus erythematous, dyshydrosis.
• Particular brief short-term applications : insect sting, parasitic prurigo after aetiological treatment.

Pharmacological Class

• Dermo-corticosteroid.

Pharmacological Properties

• Anti - Inflammatory
• Antiallergic
• Antipruritic

Mechanism of Action

• Anti-inflammatory effect : dermocorticoids antagonise vascular permeability and polymorph migration.
• Immunological effect : they act on the 4 types of immunological reactions. Their effect on type IV (allergic contact dermatitis) is the most significant in dermatology.
• Anti-micotic effect : they reduce the level of DNA synthesis and cell division. This is the mechanism of action that operates in psoriasis.

Side Effects

Telangiectasia, stretch marks, bruising purpura, skin vulnerability, drying of the skin, folliculitis, atrophy, excoriation and skin depigmentation. These effects are rare in short term treatment.

Contradictions

• Hypersensitivity to one of the components
• Primitive bacterial, viral, fungal or parasitic infection
• Ulcerous lesions.
• Application to the eyelids (risk of glaucoma)

Observations

• An increase in the number of daily applications may worsen the undesirable effects without improving the therapeutic effects.
• It is recommended that not more than 30g / week be used of this highly potent corticosteroid skin preparation.
